Aging Well is a collaborative effort in Weld County, aimed at improving services and resources for older adults in areas such as health, transportation, social inclusion, and food security. The group seeks to ensure that every older adult has the opportunities and support needed to age well and achieve self-sufficiency and dignity while aging in place. By coordinating efforts and building systems, Aging Well envisions a future where all residents can live and age happily in Weld County.
Action Plan Highlights
Transportation
The action plan aims to improve transportation for older adults and adults with disabilities by expanding wheelchair accessibility and identifying specialty transportation options. Additionally, recruiting volunteer drivers and running more public transit service on nights and weekends. Coordination with the Communications Work Group to develop a community outreach and education program about transportation resources and driver safety is included, as well as conducting mobility assessments.Social Participation
The objective is to create a stronger community through social opportunities that are inclusive for older adults. The plan aims to expand awareness and access to resources and information for older adults, families, caregivers, and community agencies/businesses. The plan also addresses barriers to inclusion and promotes intergenerational opportunities.Communication and Information
The city aims to improve access to information and resources for older adults by educating the community and organizing a resource navigation event. They also plan to assess existing systems and develop new educational programs for older adults.
