Cleveland developed its action plan through a broad coalition of stakeholders, organizing the plan around 21 strategies, with most strategies relating to more than one of the 8 domains of livability. For example, Cleveland's plan commits to more Transit Oriented Development projects as a way of enabling older adults to live independently, maintain social connectedness, and have access to amenities, healthcare services, and recreational opportunities. The plan also prioritizes investing in internet access and technology training for older adults as a way of promoting continuing education for older residents, socialization, and access to health information. Cleveland's plan is designed to reach positive outcomes whether its strategies are taken individually or together.

Determine criteria for a neighborhood to be considered an age-friendly Transit Oriented Development area, and advocate to increase the number of HUD subsidized housing units for older adults and people with disabilities in those areas.Housing
Identify home repair resources currently available and work with agencies to expand offerings for older adults, exploring formal partnerships for "wraparound" assistance, which would help residents stay in their homes through a variety of modifications.Health and Community Services
Reduce older adult fall injuries by creating partnerships with healthcare providers to refer patients to fall-prevention programs and prescribing fall-prevention activities.
