An action plan is based on the results of community surveys identifying needs within the 8 Domains of Livability that influence the health and quality of life of older adults. While there is flexibility in how a plan is created and what it will do, every action plan includes a statement about what can be achieved, target dates for goal completion, identification of participating organizations and indicators to measure progress. The action plan is an "active" rather than static document. Revisions and amendments are a sign of program improvement and progress.

Age Friendly Central Massachusetts advanced key improvements that strengthen mobility, housing stability, digital access, and community inclusion for older adults. TriValley received funding to provide long haul medical transportation. WRTA maintained fare free transit and paratransit, while partners advanced the E Bike Pilot Program and worked with visually impaired riders on WRTA Hub accessibility. The Auburn Age Friendly Pedestrian Plan and a pedestrian routes web app were completed. Regional housing capacity grew through a new Housing Coordinator, local tax abatements, and efforts to launch a homeshare program. Digital inclusion expanded with community center hotspots, a digital equity training grant, and an online ADA paratransit eligibility application. Communication improved with a new quarterly rider newsletter, and public health and safety were strengthened through community health workers and dementia related training for municipal personnel.
