Action Plan Download
Walton developed its action plan as a companion to its County Community Health Improvement Plan (CHIP), which was developed in collaboration with local hospitals and healthcare organizations, local government, community-based organizations, social service organizations, and schools. The plan highlights four primary areas of work--healthy homes, healthy places, healthy people, and healthy behaviors. Walton's work acknowledges that good health depends on homes that are safe and free from physical hazards and that the built environment is critical to older adults' physical activity. Walton also notes that specific population characteristics, including age, sex, geography, race, ethnicity and income can experience negative health outcomes, and that behavior health is essential to the health and well-being of older adults.
