Action Plan Download

An action plan is based on the results of community surveys identifying needs within the 8 Domains of Livability that influence the health and quality of life of older adults. While there is flexibility in how a plan is created and what it will do, every action plan includes a statement about what can be achieved, target dates for goal completion, identification of participating organizations and indicators to measure progress. The action plan is an "active" rather than static document. Revisions and amendments are a sign of program improvement and progress.

Action Plan Highlights

  1. Civic Participation and Employment

    Educate the local business community on dementia-friendly best practices for servicing client with memory loss, as well as employing them.

  2. Health and Community Services

    Strengthen emergency preparedness by clearly marking evacuation routes and coordinating transportation to shelters.

  3. Transportation

    Ensure residents can access town amenities by foot by providing wayfinding signage, maintaining sidewalks and improving crosswalks.

Clear Path for Veterans New England (2021)

Project Category: Public space activation

Description: As part of larger efforts to create a community space that honors veterans, Clear Path for Veterans added Adirondack chairs and tables to the Devens Memorial Courtyard. Project organizers hope the space offers visitors the opportunity to sit and quietly reflect on those who have served the nation. The new seating is also set up to facilitate private, face-to-face mental health and integrative wellness coaching sessions -- ensuring veterans and their health care providers had a safe, outdoor space to meet during the COVID-19 pandemic. Since installing the seating, Clear Path for Veterans has used the courtyard to host meetings with veterans and mental health professionals, as well as community meals and vaccination clinics.

MassBike (2022)

Worcester, MA

AARP Community Challenge Grant Year: 2022

Project Category: Bikeability

Description: To encourage intergenerational cycling, this project created a program that paired twenty teenaged cyclists with twenty older residents and awarded free e-bikes. It also held five bicycle workshops, as well as several group rides.
