Action Plan Download
An action plan is based on the results of community surveys identifying needs within the 8 Domains of Livability that influence the health and quality of life of older adults. While there is flexibility in how a plan is created and what it will do, every action plan includes a statement about what can be achieved, target dates for goal completion, identification of participating organizations and indicators to measure progress. The action plan is an "active" rather than static document. Revisions and amendments are a sign of program improvement and progress.

San Diego County's age-friendly progress report is based on the county regional plan, which is built on a collaborative, community-centered approach that identifies and addresses the regional inequities in aging. This framework is vital in meeting the needs of the growing older adult population and addresses these needs in a sustainable and equitable manner, while ensuring access to services and opportunities as our residents age. This report highlights how multiple county departments, residents, and community organizations have contributed to advancing the report's ten priority areas--health and community support, housing, social participation, transportation, dementia, caregiver support, safety, preparedness and response, silver economy, and medical and social services. With nearly 100 community partners, the county's age-friendly team emphasizes equity and community input and works to meet the needs of all older adult residents.
