An action plan is based on the results of community surveys identifying needs within the 8 Domains of Livability that influence the health and quality of life of older adults. While there is flexibility in how a plan is created and what it will do, every action plan includes a statement about what can be achieved, target dates for goal completion, identification of participating organizations and indicators to measure progress. The action plan is an "active" rather than static document. Revisions and amendments are a sign of program improvement and progress.

The City and County of San Diego worked to provide affordable transportation, address homelessness, bridge the digital divide and offer age-friendly community programming. San Diego conducted its point-in-time count -- as well as follow-up surveys -- to determine how many residents are unhoused and who they are. They also worked with community partners to prevent homelessness, advocate for affordable housing and offer home modification and repair assistance. The local transit system launched a new fare collection system and worked to teach older adults how to access reduced fares. Additionally, San Diego hosted a technology fair to educate older adults residents on telemedicine, online scams and ways technology can foster social connection. Other community events included an age-friendly film festival, as well as book discussion groups and physical fitness programming.
