Action Plan Download

An action plan is based on the results of community surveys identifying needs within the 8 Domains of Livability that influence the health and quality of life of older adults. While there is flexibility in how a plan is created and what it will do, every action plan includes a statement about what can be achieved, target dates for goal completion, identification of participating organizations and indicators to measure progress. The action plan is an "active" rather than static document. Revisions and amendments are a sign of program improvement and progress.

Action Plan Highlights

  1. Transportation

    Improve local transportation options by identifying transportation gaps and focusing on transit stops that are difficult for older adults or people with disabilities to access.

  2. Housing

    Assess the age-friendliness of current housing stock, including items like handrails, adequate lighting, easy open doors, benches, maintenance of sidewalks and parking lots, and provide age-friendly education to local landlords and leasing companies.

  3. Social Participation

    Rebuild targeted outreach opportunities including farmer's markets, faith-based organizations, and community events, and promote additional local events that prioritize social interaction and wellness for aging and diverse populations.

Description: The Chilton Public Library was looking to expand its reach beyond the walls of the library building. To give residents an intergenerational activity, library staff installed two bilingual story walks -- one at Chilton Elementary School and the other outside a local nature center. Additionally, the library created two portable story walks, which staff can set up at community events and playgrounds around Calumet County. The walks consist of several display stations installed along a trail or path. Library staff display storybook pages in each kiosk, allowing families to meander along the trail, piecing together the parts of the story as they go. A team of volunteers maintains the story walks, which display books in Spanish and English.

Project Category: Disaster Preparedness

Description: Fire, police and emergency staff will deliver preparedness guidance at a local activity center for older adults. Participants will receive emergency kits and tailored planning information.
