Action Plan Download

An action plan is based on the results of community surveys identifying needs within the 8 Domains of Livability that influence the health and quality of life of older adults. While there is flexibility in how a plan is created and what it will do, every action plan includes a statement about what can be achieved, target dates for goal completion, identification of participating organizations and indicators to measure progress. The action plan is an "active" rather than static document. Revisions and amendments are a sign of program improvement and progress.

Action Plan Highlights

  1. Housing

    Facilitate aging in place by increasing rent assistance and expanding home modification services for older adults.

  2. Health and Community Services

    Develop and distribute emergency preparation checklists, guides and kits. Conduct emergency drills for sheltering in place.

  3. Transportation

    Expand volunteer driver programs in rural communities and find ways to use ride hailing and taxi service to connect residents to public transit options.

Progress Report Download

Marin County's progress report highlights the community's work to improve transportation access and expand housing options. Marin Transit launched a voucher program, allowing older adults to access rideshare services and introduced a mobile van service to transport older adults to health and social services visits. Additionally, the County funded street infrastructure upgrades, including updated crosswalks, curb ramps and accessible parking spots. To give residents more housing options, Marin County updated its code to allow more mixed-use development. They also have seen a boom in accessory dwelling unit construction.

Project Category: Addressing community health

Description: Low-income older adults in Sonoma County often lacked access to fresh produce, relying on shelf-stable foods. Farm to Pantry addressed this by building "Farm-acy" carts stocked with locally grown fruits and vegetables, health clinics and other high-need sites. Volunteers gleaned produce from farms and added recipe cards to encourage healthy meals. The carts now provide free, high-quality food in a dignified setting, reducing waste and improving nutrition. One recipient said, "I walk out of the clinic to this bright cart full of freshly-picked oranges... it was like it was meant for me personally."

Description: This project will host six workshops to educate 120 older adults and individuals with disabilities on universal design and home accessibility solutions.
