An action plan is based on the results of community surveys identifying needs within the 8 Domains of Livability that influence the health and quality of life of older adults. While there is flexibility in how a plan is created and what it will do, every action plan includes a statement about what can be achieved, target dates for goal completion, identification of participating organizations and indicators to measure progress. The action plan is an "active" rather than static document. Revisions and amendments are a sign of program improvement and progress.

Louisville's age-friendly progress report describes community successes related to aging in place, community health and mobility and access. The local age-friendly initiative advocated for a new accessory dwelling policy, which allows homeowners to build a second housing unit on their property. Louisville increased its home maintenance services to help older adults maintain their homes. Additionally, they partnered with community organizations to offer aging-in-place resources and trainings. They worked with the Municipal Planning Organization and other partners to improve sidewalk and crosswalk accessibility and create large-print transit service maps. The City trained its municipal staff -- including emergency responders -- in age-friendly practices. They also educated residents, health care providers and City staff about opioid misuse.
