Action Plan Download

An action plan is based on the results of community surveys identifying needs within the 8 Domains of Livability that influence the health and quality of life of older adults. While there is flexibility in how a plan is created and what it will do, every action plan includes a statement about what can be achieved, target dates for goal completion, identification of participating organizations and indicators to measure progress. The action plan is an "active" rather than static document. Revisions and amendments are a sign of program improvement and progress.

Action Plan Highlights

  1. Transportation

    Improve transportation access and safety by creating educational materials on crosswalk safety, collaborating with municipalities to enhance walkability and pedestrian access, and developing an updated comprehensive transportation resource guide. Efforts also include partnering with GoGo Grandparent to expand transportation options and recruiting additional volunteers to strengthen the county's ride program.

  2. Civic Participation and Employment

    Expand civic participation and employment opportunities for older adults by creating a countywide inventory and centralized platform to promote volunteer opportunities, educating employers on job sharing and workplace flexibility, hosting in-person workshops on job readiness and skill-building, and developing a comprehensive guide to support older adult job seekers.

  3. Housing

    Enhance housing access and awareness by updating existing housing information and creating a comprehensive housing guide for distribution, launching a media campaign to highlight the benefits of affordable housing, and developing a resource guide featuring local ramp providers to support accessibility for older adults and people with disabilities.

Description: Older adults faced safety risks at home and lacked affordable options for modifications. The project delivered home safety training and distributed low-cost products, while public presentations raised awareness and encouraged practical steps. Staff guided participants on using grab bars and other aids, and outreach sparked interest in borrowing equipment from the organization's loan closet. One attendee even opened his home for volunteer-led improvements, showing how education can lead to action and safer living.
