Action Plan Download

An action plan is based on the results of community surveys identifying needs within the 8 Domains of Livability that influence the health and quality of life of older adults. While there is flexibility in how a plan is created and what it will do, every action plan includes a statement about what can be achieved, target dates for goal completion, identification of participating organizations and indicators to measure progress. The action plan is an "active" rather than static document. Revisions and amendments are a sign of program improvement and progress.

    Increase access to communication tools and information for older adults by working with community partners in the annual implementation of the Senior Expo Event, promoting the county's "one-stop-shop" Senior Services' Aging Resource Department, and continuing to distribute a comprehensive directory of county services.

Progress Report Download

Age-Friendly Forsyth County worked to implement its action plan in the wake of the COVID-19 pandemic. Its progress report describes the community's work to weather the public health crisis by improving emergency communication channels and mobilizing volunteers. Forsyth County trained residents from nonprofit organizations, faith communities and neighborhood groups to be aging advocates. Through their volunteer efforts, the age-friendly initiative distributed emergency contact info to older adults and helped them access important community resources.

Project Category: Supporting family caregivers

Description: Many older adults lacked clear, trusted sources for aging-related services. Age-Friendly Forsyth launched a volunteer-driven program that trained 46 Aging Services Ambassadors-most age 50-plus-to share resources and guidance in their communities. Volunteers learned empathetic communication and received resource notebooks and emergency "File of Life" packets for distribution. The initiative created a sustainable model for advocacy and improved awareness of local support systems. One advocate said they valued learning about the programs and services available to him, and felt empowered to help neighbors.

Project Category: Pedestrian Safety

Description: This project will install Rectangular Rapid Flashing Beacon crosswalks on Stadium Drive to enhance safety for older adults. These solar-powered signals will improve pedestrian visibility at key intersections.
