Action Plan Download

An action plan is based on the results of community surveys identifying needs within the 8 Domains of Livability that influence the health and quality of life of older adults. While there is flexibility in how a plan is created and what it will do, every action plan includes a statement about what can be achieved, target dates for goal completion, identification of participating organizations and indicators to measure progress. The action plan is an "active" rather than static document. Revisions and amendments are a sign of program improvement and progress.

Action Plan Highlights

  1. Outdoor Spaces

    Enhance the safety and accessibility of Deerfield's walking trails for older adults by publishing a comprehensive trail guide, adding occasional resting benches along key routes, improving signage and trail blazes where needed, addressing footing in challenging sections, and establishing a regular maintenance schedule to support year-round use.

  2. Transportation

    Expand transportation options for Deerfield residents by bringing the Ready Rides volunteer driver program to the community through funding support from Region 8 RCC to Region 10, identifying an accessible van service provider that meets federal requirements, and recruiting a sufficient number of volunteer drivers to ensure reliable, door-to-door transportation for older adults and people with disabilities.

  3. Housing

    Support aging in place for low-income older adult homeowners by recruiting tradespeople to provide essential home repairs, developing protocols to address liability and service eligibility, creating a directory of age-friendly tradespeople and handy persons, managing volunteers to respond to smaller or urgent needs, and updating the town's elderly property tax exemption standards to reflect current inflation, home values, and tax rates.
