Action Plan Download

An action plan is based on the results of community surveys identifying needs within the 8 Domains of Livability that influence the health and quality of life of older adults. While there is flexibility in how a plan is created and what it will do, every action plan includes a statement about what can be achieved, target dates for goal completion, identification of participating organizations and indicators to measure progress. The action plan is an "active" rather than static document. Revisions and amendments are a sign of program improvement and progress.

Action Plan Highlights

  1. Health and Community Services

    Increase the number of health care providers serving older adults by engaging medical schools and residency programs, encouraging geriatric-focused training and placements, promoting partnerships with local clinics and hospitals, and supporting incentives to attract and retain providers in the community.

  2. Communication and Information

    Promote community health and access to services by conducting public awareness campaigns on substance use, consolidating existing health and social service resource guides, and creating a singular, user-friendly guide available in both print and digital formats to help older adults and caregivers easily find and navigate available supports.

  3. Social Participation

    Expand access to inclusive programming for older adults by exploring partnership opportunities with local organizations to develop all-inclusive activities through Citrus County Parks and Recreation, increasing recreational, social, and wellness offerings that are accessible and welcoming to people of all ages and abilities.

Project Category: Public space activation

Description: Community parks lacked ADA-compliant picnic tables, leaving older adults and families without safe, comfortable places to gather. Responding to resident requests, the city installed 10 sturdy tables with signage and concrete pads across multiple parks, creating accessible spaces for rest and celebration. These upgrades encourage older adults to spend more time outdoors, walk to parks and connect with loved ones. Residents shared excitement on social media, saying they plan to visit more often now that seating is available, making Dade City parks more inviting for people of all ages.

Launched in 2017, the annual AARP Community Challenge provides small grants to local governments and nonprofits in order to fund quick-action, ready-to-implement projects that can help communities become more livable for people of all ages.
