Action Plan Download

An action plan is based on the results of community surveys identifying needs within the 8 Domains of Livability that influence the health and quality of life of older adults. While there is flexibility in how a plan is created and what it will do, every action plan includes a statement about what can be achieved, target dates for goal completion, identification of participating organizations and indicators to measure progress. The action plan is an "active" rather than static document. Revisions and amendments are a sign of program improvement and progress.

Action Plan Highlights

  1. Social Participation

    Help people age in place by implementing programs that promote connection with others so that older adults can safely leave their homes and engage with fellow community members.

  2. Transportation

    Improve access to transportation by introducing a Sand Buckets and Snow Shovel Program as well as by helping to promote and support a new program, Caribou Area Ride Service.

  3. Respect and Inclusion

    Develop and implement initiatives specific to respect and social inclusion, including a volunteer ride program technology training and a health and wellness program.

Description: Caribou faced a hidden crisis of abuse of older adults, and volunteers were without tools to educate or intervene. The group built a multimedia toolkit with expert-backed content, video vignettes and safety features to guide prevention and reporting. The resource will be shared statewide and used in local programs, strengthening community capacity to protect older adults and empowering them to speak up and access help.

Description: Bilingual home safety trainings will be offered during Acadian cultural events across Northern Aroostook County. Participants will receive safety devices and optional home assessments to support safer living.
