An action plan is based on the results of community surveys identifying needs within the 8 Domains of Livability that influence the health and quality of life of older adults. While there is flexibility in how a plan is created and what it will do, every action plan includes a statement about what can be achieved, target dates for goal completion, identification of participating organizations and indicators to measure progress. The action plan is an "active" rather than static document. Revisions and amendments are a sign of program improvement and progress.

Through its working groups, the Charlottesville Area Alliance worked to improve transportation access, expand housing options and offer residents opportunities to form social bonds. The Alliance launched a Bus Buddies program to help older adults navigate the local public transportation system. Additionally, the transportation working group engaged a consultant to conduct walk audits, which evaluated sidewalk and crosswalk accessibility. The housing workgroup offered webinars for homeowners on home modifications that enable aging in place. To combat social isolation, the Alliance also hosted a webinar series for individuals, families and service providers, which covered resources to help older adults build and maintain connections.
