Alachua County developed its action plan in alignment with the Alachua County Community Health Improvement Plan (CHIP). This allows the age-friendly work to sync with county health efforts, leverage existing resources, and maximize reach and impact. The Department of Health worked with Trust for America's Health to identify and enhance the role of public health in improving the health and well-being of the older adults in the community. Through the Florida Age-Friendly Public Health Network, the community received tools, training resources, and technical assistance from the Florida Department of Elder Affairs based on the unique needs in the county. Primary Goals: 1. Increase access to comprehensive primary care and preventive services 2. Improve access to primary care services 3. Enhance access to preventative and restorative oral health care 4. Improve access to behavioral health services 5. Reduce food insecurity 6. Improve access to affordable housing
