Description: Older adults in the rural North Conway community often faced isolation and limited access to programs that support healthy eating and social connection, especially for those living alone or on fixed incomes. Pope Memorial Library addressed this gap by building a fully functioning kitchen within its meeting space, allowing food-centered programs to take place on site rather than relying on meals prepared elsewhere. The kitchen supported nutrition education, shared meals and hands-on cooking classes that lowered barriers to participation. By cooking and eating together, older adults built routines around gathering and learning, not just attending events. Anchoring these programs in a permanent, accessible space strengthened the library's role as a reliable place for belonging and positioned it to sustain wellness programming that keeps older adults engaged over time.

Description: Among the goals of the Go, Go, Go: Making Our Community More Livable project was to enable Chinatown's residents to build friendships -- Go outdoors. Go be creative. Go make friends -- in a linguistically and culturally accessible way. One out of five Chinatown residents is an older adult and nearly 90 percent of those individuals have limited English proficiency. To help foster social connections, the Philadelphia Chinatown Development Corporation offered free art classes, with local artists serving as instructors. The organization then created a pop-up Chinatown art gallery featuring work created in the classes. In addition, PCDC founded a community garden club. More than 150 people ranging in age from 3 to 87 participated in the activities. The PCDC hopes to continue offering free community at its Crane Community Center, which broke ground in 2017 -- the first space of its kind in Chinatown.

Description: In Johnsburg, many older adults with limited mobility faced unsafe bathrooms and hard-to-navigate entrances, putting daily routines and independence at risk. With limited means to pay for fixes, hazards went unaddressed. The committee launched a pilot HomeFit Modification Program and completed improvements in seven homes using volunteers and town staff. Work included entrance and bathroom upgrades and a critical wiring repair that removed a fire risk discovered during a visit. The pilot also prompted public and private commitments to continue the program in 2026 and expand the scope so more households can receive larger safety upgrades.
