There is no shortage of underutilized urban land in Jefferson, Denver and Boulder Counties and no shortage of residents in need of fresh fruits and vegetables. Established by a group of farmers, researchers, teachers, business leaders and activists, Sprout City Farms works to increase food access and community resiliency by farming unused plots within the region. This project jumpstarted monthly workshops designed to connect residents with the local food system, along with virtual events to introduce people to Sprout City's community farming model. Additionally, the organization provided fresh produce to community members through its pay-what-you-can farm stands. The nonprofit also donated its harvest to the OUR Center in Longmont, with supplied 300 households with fruit and vegetables each week. To reach residents in their neighborhoods, Sprout City's e-bicycle-powered Sprout 'n About veggie mobiles made deliveries of fresh produce.

Description: Older adults in Roanoke lacked safe outdoor spaces for exercise and social connection, increasing isolation and health risks. The Local Office on Aging built an outdoor fitness area with specialized equipment, shaded seating and fall-resistant surfaces. Description: In rural Illinois, thousands face food insecurity and limited access to books and art. Community A-to-Z responded by installing 26 Little Free Libraries, Pantries and Art Galleries across Bureau and neighboring counties, including local community centers and food pantries. Nearly half of volunteers were age 50-plus, and building days drew more than 100 visitors, sparking additional installations beyond this project. Older adults now use the libraries as destinations for walks and bike rides, forming new connections and reducing loneliness. With plans to grow to 4,050 units, the project created a lasting network that strengthens literacy and community ties.

Description: This project created four COVID-19 Asian Response Teams to conduct outreach to community members at high risk during the pandemic. Made up of community health workers and behavior health counselors, each team's members were proficient in a different language. This allowed the Asian Health Service Center to provide over-the-phone counselling services, wellness checkups and basic emotional support in Mandarin, Cantonese, Korean, Vietnamese and English. In additions, the teams provided up-to-date COVID safety information and delivered culturally relevant groceries to participants. when vaccinations became available, the outreach teams pivoted to provide accurate information to clients in their native languages.

Description: Older adults in Denver struggle with transportation and resource navigation, limiting independence and increasing isolation. DRMAC addressed this by expanding its Getting There Travel Training program, offering bilingual sessions and guided bus trips to teach trip planning, ticketing and mobility apps. Surveys showed that many participants felt more confident using public transit. The effort strengthened trust in marginalized communities and improved access to daily needs. One participant said that as a result of the help provided by the program, he is excited to now be able to attend spirit-lifting activities.

Description: Older adults and people with disabilities in Denver face cracked sidewalks, missing curb ramps and unsafe crossings on the way to bus stops. Denver Regional Mobility and Access Council led a walk audit engaging 50-plus residents, including 28 older adults, to document hazards with a structured toolkit. One participant with limited mobility said they had avoided certain streets and felt hopeful that "real improvements could be made" once planners saw the data. The audit produced actionable findings for agencies to prioritize repairs and strengthened partnerships and advocacy so residents can press for safer walking routes to transit over time.
