Description: Transportation barriers kept many older adults in Baltimore from attending medical appointments, increasing isolation and health risks. The Chaperone Program recruited volunteers and launched outreach campaigns, including billboards and social media, to connect patients with trained companions for rides and campus navigation. The initiative also provided telephonic support and durable medical equipment like wheelchairs and walkers. One patient shared, "With a chaperone, I made every rehab appointment and got my prosthesis." The program aims to expand across additional Ascension sites.

Project Category: Park enhancements

Description: To allow for more community events in Veterans Memorial Park, the City of Central Falls purchased two canopies to shade visitors from the sun. The City then set up the all-weather canopies at a series of events held in the park, including a picnic, Zumba gatherings and ice cream socials. Project organizers say the canopies will allow them to expand outdoor programming aimed at older adult residents.
