Action Plan Download

An action plan is based on the results of community surveys identifying needs within the 8 Domains of Livability that influence the health and quality of life of older adults. While there is flexibility in how a plan is created and what it will do, every action plan includes a statement about what can be achieved, target dates for goal completion, identification of participating organizations and indicators to measure progress. The action plan is an "active" rather than static document. Revisions and amendments are a sign of program improvement and progress.

Action Plan Highlights

  1. Communication and Information

    Enhance communications capabilities by creating an all-ages resource directory to increase awareness of existing resources and services available to residents.

  2. Health and Community Services

    Improve cognitive and social engagement by providing coordinated affordable fitness activities geared toward older adults that also provide multi-generational interaction.

  3. Respect and Inclusion

    Enhance local multi-generational programs connecting older adult residents and college students through fitness and movement.

Description: The program will improve unsafe front-yard walkways for older adults by removing hazards, clearing paths and adding simple visibility features. These changes will help residents travel safely between their homes and neighborhood services.
