Action Plan Download
An action plan is based on the results of community surveys identifying needs within the 8 Domains of Livability that influence the health and quality of life of older adults. While there is flexibility in how a plan is created and what it will do, every action plan includes a statement about what can be achieved, target dates for goal completion, identification of participating organizations and indicators to measure progress. The action plan is an "active" rather than static document. Revisions and amendments are a sign of program improvement and progress.

The objectives include finding solutions for healthier and accessible neighborhoods, addressing housing issues, expanding the Home Repair Program, improving emergency preparedness, raising community awareness, and providing assistance with yard work and snow shoveling for residents who cannot manage these tasks independently.Respect and Inclusion
The objectives include creating intergenerational opportunities with schools, implementing "buddy benches" for personal connections, engaging with Home Delivered Meal recipients for social engagement, researching best practices for connecting with mentally challenged individuals, and improving digital navigation classes in collaboration with community partners.Communication and Information
The goals include creating a newsletter for residents with age-friendly resources, reviving the Block Watch Program to share information, implementing OATS (Older Adults Technology Services) programming for older adults, and improving technology services at the Hart Park Senior Center.
