An action plan is based on the results of community surveys identifying needs within the 8 Domains of Livability that influence the health and quality of life of older adults. While there is flexibility in how a plan is created and what it will do, every action plan includes a statement about what can be achieved, target dates for goal completion, identification of participating organizations and indicators to measure progress. The action plan is an "active" rather than static document. Revisions and amendments are a sign of program improvement and progress.

Since completing its age-friendly action plan, the City of Sisters has achieved successes related to mobility, pedestrian safety, housing diversity and combatting social isolation. After launching a rideshare program, hundreds of older adults have secured transportation to medical appointments. Additionally, streetscape improvements -- such as speed humps and new crosswalks -- have improved pedestrian safety. To encourage missing middle housing construction, Sisters modified its zoning to allow duplexes, townhomes, accessory dwelling units and cottage clusters, as well as manufactured housing options. Additionally, the city offered education and community events to support people with dementia and their caregivers.
