Action Plan Download

An action plan is based on the results of community surveys identifying needs within the 8 Domains of Livability that influence the health and quality of life of older adults. While there is flexibility in how a plan is created and what it will do, every action plan includes a statement about what can be achieved, target dates for goal completion, identification of participating organizations and indicators to measure progress. The action plan is an "active" rather than static document. Revisions and amendments are a sign of program improvement and progress.

Action Plan Highlights

  1. Transportation

    Expand community mobility options to support older adults by introducing a fixed-route shuttle service connecting key points of interest for improved access to social and recreational activities, piloting an affordable rideshare program for on-demand senior transportation, investing in pedestrian infrastructure upgrades such as safer crosswalks, enhanced lighting, and added seating along key walking routes, and conducting public safety workshops to teach safe driving techniques and defensive walking habits to prevent vehicle collisions.

  2. Health and Community Services

    Strengthen support services for older adults by developing a service navigator program to connect seniors with available resources, launching an annual Nutrition Outreach initiative to increase awareness of senior meals programs, and introducing senior-specific emergency preparedness workshops that provide clear, accessible guidance on creating emergency plans and assembling essential supplies.

  3. Social Participation

    Foster neighborhood-based social connections among older adults by implementing a Village-to-Village model that organizes seniors to participate in mutual support networks and community social activities, encouraging peer-to-peer assistance, and strengthening a sense of belonging within local neighborhoods.
