Action Plan

An action plan is based on the results of community surveys identifying needs within the 8 Domains of Livability that influence the health and quality of life of older adults. While there is flexibility in how a plan is created and what it will do, every action plan includes a statement about what can be achieved, target dates for goal completion, identification of participating organizations and indicators to measure progress. The action plan is an "active" rather than static document. Revisions and amendments are a sign of program improvement and progress.

Action Plan Highlights

  1. Transportation

    Focus on transportation for older adults by conducting a walkability survey, hosting travel trainings, and identifying resources for older adults on alternative transportation.

  2. Communication and Information

    Create additional methods of communication with older adults including new information boards throughout the city and a community newsletter.

Progress Report Download

The City of Garfield's age-friendly progress report focuses on social opportunities and inclusivity. To help residents feel included, Garfield hosted a variety of activities and events, including arts and crafts gatherings and food and music festivals. Additionally, they hosted educational sessions on topics such as fraud prevention. They also worked to inform residents about resources, such as the local food pantry, community health center and assistance programs.

Project Category: Addressing community health

Description: Through its Generations for Garfield initiative, the City of Garfield set up pop-up wellness centers at its local VFW post. Part of a pilot program, the centers provided a centrally located, multigenerational space for programming, including arts, cooking and exercise classes. Since the pilot, the effort culminated in construction of the Garfield Engagement Center in 2019.

Project Category: Addressing community health

Description: This project will help create a community pantry by installing shelving, refrigeration units and an accessible ramp. The pantry will simulate a grocery shopping experience, allowing visitors to browse the aisles, select food items and check out with program staff.
