Action Plan Download

An action plan is based on the results of community surveys identifying needs within the 8 Domains of Livability that influence the health and quality of life of older adults. While there is flexibility in how a plan is created and what it will do, every action plan includes a statement about what can be achieved, target dates for goal completion, identification of participating organizations and indicators to measure progress. The action plan is an "active" rather than static document. Revisions and amendments are a sign of program improvement and progress.

Action Plan Highlights

  1. Transportation

    Assess the long-term viability of the SMART transit program, o ering free rides for older adults. Educate older adults about the transportation options available to them. Expand ride service availability within Freeborn County.

  2. Health and Community Services

    Update and expand on existing programs, including the Reassurance Call-In Program. Provide or develop opportunities focused on in-home care services and support. Research funding and grant opportunities to support the sustainability of the Lake Song Chorus and Memory Cafe.

  3. Civic Participation and Employment

    Facilitate informational quarterly presentations open to the public on various topics. Expand informational presentations into rural areas. Educate businesses regarding employment options for older adults. Curate a database of volunteer opportunities throughout Freeborn County that contains detailed information.
