Clinton County developed its action plan focusing on the unique health concerns of older adults in a largely rural community. The plan looks at creating more reliable transportation for medical specialists located outside the county, and combating social isolation in the context of mental health and the acceleration of dementia. The action plan also works to help older adults age in place by offering more quality, diverse, and affordable housing options for older residents and providing access to home improvement contractors who are trained in accommodating the needs of older adults. Additionally, Clinton County's plan works to ensure that new housing options for older residents are well-integrated in the community and not isolated on the fringe, so as to allow for greater engagement between residents of all ages.
