Action Plan Download

An action plan is based on the results of community surveys identifying needs within the 8 Domains of Livability that influence the health and quality of life of older adults. While there is flexibility in how a plan is created and what it will do, every action plan includes a statement about what can be achieved, target dates for goal completion, identification of participating organizations and indicators to measure progress. The action plan is an "active" rather than static document. Revisions and amendments are a sign of program improvement and progress.

Action Plan Highlights

  1. Transportation

    Develop multilingual transportation guides explaining eligibility requirements, scheduling procedures, service availability, and contact information. Conduct transportation workshops in senior housing buildings, community centers, and libraries. Explore development of scheduled shuttle services for older adults attending healthcare appointments and community programs. Increase awareness of transportation assistance programs through targeted outreach campaigns.

  2. Communication and Information

    Launch citywide outreach campaigns using flyers, phone calls, email communication, local newspapers, and social media platforms. Partner with faith-based organizations, senior housing buildings, healthcare providers, and community organizations to distribute information. Establish regular information sessions in accessible community locations. Create a centralized senior resource directory listing available health and social support services. Recruit local residents, including older adults, to serve as volunteer Community Ambassadors. Train ambassadors on transportation services, health resources, referral procedures, and community navigation. Conduct door-to-door outreach and face-to-face engagement in senior residential buildings. Assist older adults with service enrollment and resource navigation.

  3. Health and Community Services

    Provide wellness education programs focusing on chronic disease prevention, nutrition, exercise, and mental health. Increase access to preventive health screenings through community health fairs. Connect older adults to healthcare navigation services. Provide medication management and chronic disease self-management education. Promote healthy aging campaigns emphasizing independence and preventive care.

Walk Bike Ride South Orange with Meadowland Park Conservancy (2025)

South Orange, NJ

Project Category: Bikeability

Description: The Conservancy addressed safety concerns on a street that many older adults use to reach the park and community center. Mead Street also cuts through a natural corridor where turtles cross between water sources, a reminder that poor visibility and fast traffic created risks for people on foot and on bikes alike. The project installed brightly colored, artistic crosswalks featuring turtles, applied durable green paint to bike lanes and added flexible delineators that shorten crossing distances and protect cyclists. Corrider users already report easier, more confident access to park amenities and programming. The demonstration features are now being evaluated for permanent use and are informing broader street safety planning, giving the village a tested model for improving walkability and bike safety in future projects.

Launched in 2017, the annual AARP Community Challenge provides small grants to local governments and nonprofits in order to fund quick-action, ready-to-implement projects that can help communities become more livable for people of all ages.
