Action Plan Download

An action plan is based on the results of community surveys identifying needs within the 8 Domains of Livability that influence the health and quality of life of older adults. While there is flexibility in how a plan is created and what it will do, every action plan includes a statement about what can be achieved, target dates for goal completion, identification of participating organizations and indicators to measure progress. The action plan is an "active" rather than static document. Revisions and amendments are a sign of program improvement and progress.

Action Plan Highlights

  1. Outdoor Spaces

    Renovate Bartlem Park to make it more inclusive for community members, enabling the space to host summer concerts, festivals and other activities.

  2. Communication and Information

    Offer group and individual digital literacy training at the local library and senior center, as well as for public housing residents.

  3. Transportation

    Expand on-demand transportation services for older adults and people with disabilities by expanding the community's fleet of accessible vehicles.

Description: The Mill River Underpass Park Project will reconnect three neighborhoods divided by I-91 by creating an intergenerational, multi-use space. It will prioritize nature, accessibility, and communal areas, helping older residents safely cross between neighborhoods and rebuild social connections.

Project Category: Public space activation

Description: New Haven's older adults lacked outdoor seating, leaving older adults without safe, accessible places to rest or socialize at local community centers. The department installed ADA-compliant benches at three centers and created mental health resource materials for distribution. Volunteers guided bench placement and unveiling events. The project improved accessibility, encouraged social interaction and sparked collaboration between city departments. Feedback praised the benches as "a good way to have conversation and fellowship" and "great when you are waiting for your ride."

Launched in 2017, the annual AARP Community Challenge provides small grants to local governments and nonprofits in order to fund quick-action, ready-to-implement projects that can help communities become more livable for people of all ages.
