Action Plan Download
Alexandria developed its action plan around three mission-driven categories. The plan focuses on promoting physical wellbeing throughout a lifetime, with optimal access to healthcare in its various forms and affordability; working with community partners to provide products and services that foster safe and livable homes and communities, and that help people of all ages achieve the strength, stability and self-reliance they need to build better lives for themselves; and to expand transportation services and options, as well as to increase the overall transit demand, capacity, accessibility, affordability, reliability and efficiency.
Action Plan Highlights
Health and Community Services
Create a home visitation program comprised of trained volunteers who can follow up with patients who have been released from the hospital, helping prevent hospital readmission.Transportation
Share safety information with older drivers and their families by designing and implementing a targeted public service campaign.
Progress Report Download
Alexandria's age-friendly progress report focus on three primary programs. First, the city's dementia-friendly initiative established projects in different communities and will be working with many rural communities over five counties. Next, the community focused on disaster resilience, with the Emergency Management Department offering many educational presentations and helping the community after tornado devastation destroyed many homes. Finally, the Connecting Communities initiative is working to ensure that any time an older adult or their caregivers has a question or problem, it will be addressed. Alexandria's age-friendly program is run entirely by volunteers and is entirely grant and partner funded.
