Action Plan Download
Walton developed its action plan as a companion to its County Community Health Improvement Plan (CHIP), which was developed in collaboration with local hospitals and healthcare organizations, local government, community-based organizations, social service organizations, and schools. The plan highlights four primary areas of work--healthy homes, healthy places, healthy people, and healthy behaviors. Walton's work acknowledges that good health depends on homes that are safe and free from physical hazards, and that the built environment is critical to older adults' physical activity. Walton also notes that specific population characteristics, including age, sex, geography, race, ethnicity and income can experience negative health outcomes, and that behavior health is essential to the health and well-being of older adults.Healthy People Disparities in health outcomes exist in specific groups of people. When exploring preventable injuries, chronic diseases, and infectious diseases, specific populations are more at-risk or vulnerable. Defining characteristics of these populations include age, sex, geography, race, ethnicity, or income.Healthy Behaviors Behavioral health is essential to a person's overall health and wellbeing, interpersonal relationships, and ability to live a full and productive life. Mental illness can influence the onset, progression, and outcome of other illnesses and often correlates with health risk behaviors such as substance abuse, tobacco use, and physical inactivity. These priorities guided the development of goals, objectives, and key activities that will be implemented to promote optimal health and well-being for all who live, work, learn, play, and pray in Walton County. The result of the MAPP process is a well-crafted roadmap that we will review and revise annually to meet emerging challenges and opportunities.
